Required paperwork is available to be picked up in advance at the Doddridge Community Center or in Texarkana at the College Hill Pharmacy. Please arrive with your driver’s license, insurance card and completed paperwork.

College Hill Pharmacy in Texarkana.
Please arrive for your COVID-19 immunization shot with your driver’s license, insurance card and your completed paperwork.
